Electronic Visa Application Forms

ELECTRONIC VISA APPLICATION FORMS
As of November 1, 2006 all applicants who wish to apply for any category of nonimmigrant visa to the United States at the U.S. Consulate General in Amsterdam must complete the visa application form on the internet using the Electronic Visa Application Form, or EVAF.
This form is available on http://evisaforms.state.gov and must be completed online and then printed with a clearly legible barcode.
The requirement for use of the EVAF is being implemented at all embassies and consulates worldwide in an effort to enhance both the security and efficiency of the visa application process.
Nonimmigrant visa applications submitted with a handwritten or typewriter typed form (called the DS-156) can no longer be accepted. All applicants who submit a handwritten or typewriter typed DS-156 application form will be asked to complete and print out the new electronic version of the application form. Visa application forms printed from the internet and then filled out will not be accepted.
